DHR Health in Edinburg, TX is seeking an RN Case Manager to advocate for hospital clients, coordinate care, and manage complex patient needs to ensure quality and cost-effective outcomes across the continuum.

Candidates must be a graduate of an accredited nursing program with a Texas RN license; CCM is desired. Three to five years acute care experience; bilingual English/Spanish; on-site role with weekend rotation and on-call.

Qualifications

• Graduate from an accredited school of nursing.
• Current Texas RN license required.
• CCM certification highly desired.
• Candidate must demonstrate proficiency in English and Spanish.
• Three to five years clinical experience; hospital/acute care preferred.
• Knowledge of Medicare/Medicaid guidelines and utilization review.

Responsabilités

• Assist in the development and implementation of the case management program.
• Collaborate with departments to ensure appropriate resource utilization.
• Work with nurse managers and care team to move patients through the system.
• Develop discharge plans early in admission and coordinate referrals.
• Review medical records to determine medical necessity and plan care.
• Provide ongoing education and act as a consultant on case management.
