TRIUNE Health Group in Van Nuys, CA seeks a Nurse Case Manager to coordinate medical case management for catastrophically or chronically ill or injured individuals, aiming for quality, holistic treatment and timely return to work. The role requires RN licensure in California, a graduate nursing degree, strong communication and leadership, plus 3+ years clinical experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in California.
• Fluency in English (speaking, reading, and writing).
• CCM certification preferred.
• Three or more years of recent clinical experience, preferably in trauma, psychology, emergency, orthopedics, rehabilitation, occupational health, and neurology.

Responsabilités

• Provide medical case management coordinating with patient, physicians, other health care providers, employer, and referral source.
• Utilize Case Management steps to assess, plan, implement, evaluate and report on progress.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
• Coordinate delivery of durable medical equipment and home health services with clear communication.
• Assess rehab facilities and coordinate transportation and home assessments as needed.
• Communicate medical information clearly to patients and families.
• Stay current with health-care laws (HIPAA, FMLA, ADA, Workers' Comp, etc.).
• Use technology to prepare timely reports in compliance with safety rules and HIPAA.
