Job summary

OptumCare Hampden & Hampshire Counties, MA is seeking a Nurse Practitioner to perform annual in-home health assessments for Medicare Advantage and other health plan members. This field-based role focuses on preventive care, patient education, and improving health outcomes in collaboration with primary care providers.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active and unrestricted NP license in practicing state
• Active driver's license with reliable transportation for home visits
• Spanish bilingual and eligible to practice in MA

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other members
• Identify and close care gaps in collaboration with primary care providers
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect patients with resources
• Operate in an independent field-based role with interdisciplinary support
