Job summary

Titanium Healthcare is seeking a LVN Care Coordinator to join our ECM team. This hybrid field-based role combines in-person member visits with administrative work, traveling within the assigned service area while maintaining a flexible schedule.

You will support access to primary and behavioral health care, engage with an interdisciplinary team, and help coordinate care under IEHP requirements. A valid LVN license and 2+ years of experience are required.

Qualifications

• Diploma/GED required.
• A valid, unrestricted state LVN license.
• Minimum 2+ years of experience as an LVN in a healthcare setting.
• Prior experience using EHR/EPM systems.
• Current and valid Driver’s License with a clean driving record.

Responsibilities

• Facilitate access to primary care and behavioral health providers.
• Collaborate with Titanium’s interdisciplinary team to provide high-quality care management.
• Coordinate care and resource linkages to ensure quality of care for ECM members.
• Perform in-person visits and follow-up appointments as scheduled.
• Document activities and maintain compliance with policies and procedures.
