Zusammenfassung

Titanium Healthcare seeks a Care Coordinator to manage patient care, scheduling, and coordination with a healthcare team. The role involves both remote work and field visits in King County, with 30% remote work and 70% travel for in-person member visits, plus mileage reimbursement.

You will educate members and families about treatments and resources, maintain documentation in the EHR system, and ensure timely communication with providers.

Qualifikationen

• Fluent in English (written and verbal).
• Experience documenting care and coordinating services.
• Ability to communicate with diverse healthcare teams.
• Strong organizational and detail-oriented skills.

Aufgaben

• Maintain caseload of up to 50 and conduct monthly in-person visits.
• Review eligibility for enrollment in Health Homes Program (HHP).
• Complete referrals and educate members about care options.
• Coordinate care plans with healthcare providers and teams.
• Document care activities accurately in the EHR system.
• Schedule check-ins and manage member encounters and reporting.
