Job summary

Medix is seeking an LVN Care Coordinator in Garden Grove, CA for a hybrid role. You will support Enhanced Care Management transitions, coordinate follow-up care, and reinforce discharge instructions under RN supervision.

You will use EHR workflows, gather medication lists, arrange appointments, and help patients access services. Office-based with remote elements and a strong emphasis on privacy and coordination.

Qualifications

• Completion of a LVN program and licensure or eligibility.
• Experience documenting in an EHR system.
• Ability to work from the Garden Grove, CA office regularly.

Responsibilities

• Coordinate follow-up with ECM members during care transitions.
• Gather clinical information and reinforce discharge instructions.
• Support medication reconciliation and access to services.
• Schedule follow-up appointments and coordinate transportation and services.
• Maintain accurate documentation in designated systems.
