Elevance Health seeks a LTSS Service Coordinator (Bilingual EN/ES) to manage a defined caseload, conduct assessments, and develop Person Centered Plans in collaboration with the member and care team.

The role is field-based, serving Lee/Sarasota counties primarily, with hybrid office options subject to policy and commuting distance requirements; requires a BA/BS and 2+ years in social services.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience with a social work agency.
• Education and experience must meet state law and contract requirements.
• Preferred bilingual English/Spanish proficiency.

Responsibilities

• Conduct face-to-face program assessments using predefined questions.
• Coordinate members' waiver cases (LTSS/IDD) and BH/PH needs.
• Document goals in PCSP and backup plans with the care team.
• Identify members who may need alternative levels of service.
• Submit utilization/authorization requests with care plan support.
• Report critical incidents to state/county agencies when required.
• Assist with appeals, grievances, and state audits.
