Job summary

SGS Consulting is seeking a Service Coordinator to conduct home visits and coordinate care for LTSS participants. You will collaborate with the member, authorized representative, PCP, and care team to ensure safe discharge and appropriate services.

A key responsibility is documenting in the electronic health record and developing cost-effective care plans. The role requires a Bachelor's degree or non-licensed master-level clinician with relevant fields and at least 2 years of field case

Qualifications

• 2 years field case management experience preferred.
• Bilingual preference (Spanish).
• Bachelor's degree or non-licensed master level clinician required, degree in psychology, social work, counseling, sociology, human growth and development, or early childhood education.

Responsibilities

• Conduct face-to-face home visits using assessment tools for LTSS participants.
• Coordinate and collaborate care with member/authorized representative, PCP, and care team.
• Schedule and attend interdisciplinary meetings and advocate for safe discharge with appropriate services.
• Develop a care plan and authorize services cost-effectively within LTSS benefits.
• Document accurately and timely in the member's electronic health record.
