Job summary

Qualifications

• BA/BS degree required; 2+ years experience in social work or equivalent.
• Experience with LTSS/IDD, case management, or discharge planning preferred.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Perform face-to-face program assessments using predefined tools and motivational interviewing.
• Coordinate cases with clinical teams to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses to ensure efficient LTSS utilization.
• Document goals and plans with members and care teams.
• Identify members who may benefit from alternative service levels or other waivers.
• Mentor or train new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate agencies and support audits.
