Job summary

Chicanos Por La Causa, Inc. is seeking a licensed LPN to provide integrated health services at our Corazón program in Phoenix.

You will coordinate care with community providers, assist with admissions and medication management, and respond to emergencies with professionalism in a dynamic setting. The role requires an active LPN license in Arizona, a minimum of one year of medical experience, and flexibility to work evenings and weekends as needed.

Qualifications

• Current LPN license issued by the Arizona State Board of Nursing.
• Minimum 1 year experience performing medical duties.

Responsibilities

• Provide integrated health and support services and coordinate care with community providers.
• Educate clients on mental and medical health and assist with medication management.
• Respond promptly to medical emergencies and maintain licensure and certifications.
• Support program participation and schedule medical or provider appointments as needed.
• Work flexible hours including evenings, nights, and weekends to meet client needs.
