Job summary

Community Bridges, Inc. (CBI) is seeking a full-time Licensed Practical Nurse (LPN) to provide integrated behavioral health care across crisis and inpatient units.

The LPN will join a multidisciplinary team and follow orders from the medical practitioner, communicating barriers to service promptly to the RN and attending physician. The role requires an accredited nursing degree with an Arizona LPN license and a BHT credential per policy; 5 weeks PTO, comprehensive benefits, and competitive pay

Qualifications

• Associate degree from an accredited nursing school is required.
• 1–3 years of health care experience with exposure to behavioral and medical crisis situations is preferred.
• Arizona Board of Nursing license as an LPN is required and in good standing.
• BHT per A.A.C. R9-10-101.33 and CBIs policies.
• Valid identification is required.
• Arizona or Compact Board license as Practical Nurse must be current and in good standing.

Responsibilities

• Provide delegated nursing care as part of the multidisciplinary team under the RN on duty.
• Carry out orders from the medical practitioner and communicate barriers promptly.
• Provide integrated behavioral health care on crisis and inpatient units.
