Job summary

AvoCare is seeking a compassionate Licensed Practical Nurse (LPN) for part-time palliative care alignment in Glendale, AZ. Qualifications

• Currently licensed as a Practical Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

Responsibilities

• Provide skilled nursing care and ongoing support to patients living with serious illnesses in the home and other care settings.
• Asses patients, monitor symptoms, administer medications as directed, educate patients and families, and collaborate with the care team to ensure high-quality care.
• Document visits in EMR, maintain HIPAA compliance, and communicate with team and case managers.

Education
