AvoCare is seeking a compassionate Licensed Practical Nurse (LPN) for part-time palliative care alignment in Glendale, AZ. Qualifications

Currently licensed as a Practical Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

Responsibilities

Provide skilled nursing care and ongoing support to patients living with serious illnesses in the home and other care settings.
Asses patients, monitor symptoms, administer medications as directed, educate patients and families, and collaborate with the care team to ensure high-quality care.
Document visits in EMR, maintain HIPAA compliance, and communicate with team and case managers.
