Job summary

LCH Health & Community Services is seeking a Licensed Practical Nurse (LPN) to support patient care through chronic disease management, triage, and care coordination across primary care, women’s health, and pediatrics.

The role requires bilingual English/Spanish capabilities, an LPN license in Pennsylvania, CPR certification, and experience with EHR systems. Travel between LCH locations is expected for full-time coverage.

Qualifications

• Bilingual English/Spanish required.
• Associate or bachelor’s degree in nursing.
• Current or eligible LPN license in Pennsylvania.
• CPR certification required.
• Experience with diverse populations preferred.
• Community/public health experience preferred.

Responsibilities

• Provide triage support (phone and in-person).
• Manage chronic care patients and support population health initiatives.
• Educate patients and families on chronic conditions and treatment plans.
• Coordinate follow-ups, referrals, and transitions of care.
