Qualifications

• Completion of nursing training from an accredited school of nursing.
• 2 years of experience as an LPN; experience with EHR systems.
• Current, active, and unrestricted LPN license in AZ.

Responsibilities

• Collaborates with prescribing clinicians and clinical staff to promote member Recovery.
• Completes medical procedures and makes necessary referrals; coordinates medical care.
• Monitors members’ medical symptoms, reports assessments for follow-up.
• Provides medication delivery as ordered by the prescribing clinician.
• Performs charting and record-keeping per agency requirements.
• Coordinates education to identified medical needs; wellness support.
• Administers injections and participates in meetings and staffing.
