Senior Care Therapy, LLC, based in Elizabeth, NJ, is seeking a Bilingual Licensed Professional Counselor (LPC) to join our geriatric in-person psychotherapy team in SNFs across NJ, NY, PA, and MD.

This full-time role offers work-life balance, a competitive salary ($56k–$90k annually), sign-on bonus up to $6,000, and comprehensive benefits including health, dental, vision, 401(k) with match, PTO, and CEU reimbursement. You will provide individual and group therapy at assigned facilities.

Qualificações

• Active and unrestricted Licensed Professional Counselor license for the state applying for.
• Excellent oral and written communication in English and Spanish.
• Proficiency with electronic health records (EHR) and general tech readiness.

Responsabilidades

• Conduct individual psychotherapy sessions for geriatric patients in subacute or SNF settings.
• Conduct group psychotherapy sessions as appropriate.
• Develop and implement evidence-based treatment plans for each patient.
• Collaborate with facility staff, interdisciplinary teams and families to support care.
• Maintain accurate clinical documentation per regulatory standards in the EHR.

We currently serve over 300 Skilled Nursing Facilities including sub-acute, long-term care, and assisted livings throughout NJ, NY, PA, and MD.

Key Responsibilities:

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
• Develop and implement evidence-based treatment plans tailored to the needs of each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
• Monitor patient progress and adjust treatment plans as needed.

Requirements:

• Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
• Effective oral and written communication in English and in Spanish
• Basic proficiency with technology, including electronic health records (EHR).
• Strong organizational and documentation skills, with attention to regulatory compliance.
• Ability to provide in-person services at assigned facilities, up to 45 minutes.
• Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
• Full Vaccination and Booster Status may be required in some facilities.

Preferred Qualifications:

• Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
• History of treating Anxiety, Depression and Adjustment Disorders.
• Psych or Addictions experience is a plus.
• Excellent time management and organizational skills.
