The role emphasizes autonomy with flexible daytime hours and collaborative care with clinical teams.

You will conduct individual and group sessions, develop evidence-based treatment plans, document accurately in the PacEHR EMR, and participate in ongoing training and mentorship.

Qualifications

• Active and unrestricted LPC license in the state applying for.
• Fluent in English and Spanish.
• Proficient with EHR systems and basic technology.
• Strong organizational and documentation skills with regulatory focus.
• Willingness to provide in-person services at facilities, up to 45 minutes.
• Ability to move between patient rooms and offices throughout the day.
• Vaccination and boosters may be required in some facilities.

Responsibilities

• Conduct individual and group psychotherapy sessions in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans.
• Collaborate with facility staff, interdisciplinary teams, and families.
• Maintain accurate clinical documentation in the EMR.
• Monitor patient progress and adjust treatment plans.
