Job summary

Serve-the-People Community Health Center is seeking a Licensed Clinical Social Worker (LCSW) in California to work with medical and behavioral health teams. You will conduct biopsychosocial assessments, develop treatment plans, coordinate care, and provide crisis intervention when needed.

The role requires CA LCSW licensure, a minimum of 1 year post-master’s clinical experience, and strong communication skills. Spanish language ability is preferred to serve the community effectively.

Qualifications

• Licensed LCSW in California with 1 year post-master’s clinical experience.
• Excellent verbal and written communication, detail oriented, able to work independently.
• Must have a valid NPI and CPR/BCLS certification.

Responsibilities

• Conduct intake, treatment planning, and ongoing clinical treatment for patients.
• Follow state and federal guidelines for care provision.
• Collaborate with clinical and non-clinical staff for proactive, coordinated care.
• Communicate professionally, enforce policies, and work under pressure.
• Develop biopsychosocial assessments and measurable intervention plans.
• Document progress notes and treatment plans timely and concisely.
• Provide crisis intervention and coordinate care with primary care staff.
