Job summary

Community Health Centers of America (CHCA) in California seeks a Licensed Clinical Social Worker (LCSW) to join our outpatient primary care team in Fresno. This full-time role provides integrated behavioral health services, psychosocial assessments, crisis intervention, and short-term therapy for an underserved population.

The ideal candidate holds an MSW from a CSWE-accredited program, an active California LCSW license, bilingual English/Spanish, and EMR/care coordination experience.

Qualifications

• MSW from CSWE-accredited program.
• Active, current LCSW license in California.
• Minimum 1 year post-licensure experience preferred.
• Bilingual English/Spanish preferred.
• Familiarity with EMR systems and care coordination.

Responsibilities

• Provide integrated behavioral health services within outpatient primary care visits.
• Conduct psychosocial assessments and develop short-term treatment plans.
• Deliver brief, evidence-based psychotherapy and crisis stabilization.
• Coordinate with care managers and physicians to address SDOH needs.
• Maintain compliant documentation in the EHR and ensure regulatory standards.
