Job summary

Delta-T Group is seeking a Housing Case Manager in Boston, starting immediately. You will interview shelter clients, assess needs, and begin case management while maintaining HMIS records and timely documentation. Build individualized plans, monitor progress, and coordinate with housing programs and partners to secure housing.

The role requires 1–2 years in health and human services with the homeless population, bilingual skills preferred, and the ability to obtain crisis intervention training.

Qualifications

• College degree or high school diploma/GED, depending on experience.
• 1–2 years' experience in health and human services work with homeless population.
• Haitian or Spanish language skills encouraged to apply.
• Basic proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.
• Must be able to obtain and maintain crisis intervention and complete any required training.
• Individuals experienced with housing insecurity and homelessness are strongly encouraged to reply.

Responsibilities

• Interview, assess, and begin case management for shelter clients who have been in the shelter for at least five days.
• Complete intakes, assess guests' needs, and identify factors leading to shelter entry and eligibility for resources.
• Develop individualized case plans with goals and monitor progress via weekly meetings.
• Maintain electronic client records in HMIS and complete required documentation promptly.
• Provide referrals and assist with applications to housing programs and services.
• Establish relationships with community partners and landlords to secure and maintain housing.
