Job summary

Delta-T Group in Boston, MA is seeking a Bachelor Level Case Manager for an immediate start on a temporary assignment. The role requires strong clinical skills, adherence to documentation, and experience in youth-related case management within the Massachusetts child welfare context.

The position offers on-site work in Boston with opportunity to engage families, plan transitions, and participate in discharge planning while coordinating with residential staff and supervisors.

Qualifications

• Bachelor's degree in social work or human services field
• At least 3 years of clinical experience
• Familiarity with Massachusetts child welfare systems and/or DCF
• Must have valid driver's license and vehicle

Responsibilities

• Provide clients with clinical services, including case management, individual treatment or group work
• Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans
• Oversee the intake process
• Determine appropriate transition and discharge plans for all residents
• Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns
• Facilitate case conferences and discharge planning for all residents
• Assist with family engagement, permanency planning and life-skills development for all residents
• Provide on-call support
