Job summary

Delta-T Group seeks a Housing Case Manager in Lowell, MA to manage shelter guests’ needs through comprehensive intake, assessment, and case planning. This independent contractor role involves weekly meetings, HMIS documentation, and coordination with housing providers to secure and maintain housing.

Responsibilities include referrals to housing programs and benefits, problem-solving for barriers, record-keeping, and ongoing contact with community partners to support guest goals.

Qualifications

• High school diploma or college degree required depending on experience.
• 1–2 years in health and human services with homeless population.
• Ability to obtain and maintain crisis intervention training.
• Bilingual in Haitian Creole or Spanish encouraged.

Responsibilities

• Interview, assess, and begin case management for shelter guests after five days of shelter stay.
• Complete intakes, assess needs, identify resources, and determine eligibility for services.
• Develop individualized case plans with goals and monitor progress via weekly meetings.
• Maintain electronic client records in HMIS and complete required documentation promptly.
• Provide referrals and assist with applications to housing programs and benefits based on guest goals.
• Establish and maintain relationships with community partners and housing providers to secure and sustain housing.
