Job summary

Acacia Network is seeking a Service Coordinator (Homes Together Program) in Bronx, NY to provide housing support and case management under the Program Director. Responsibilities include liaison work with building managers, on-site crisis intervention, and documentation of monthly progress and program data.

The role requires an Associate’s Degree and 3+ years in mental/behavioral health, with strong organizational and communication skills.

Qualifications

• Associate’s Degree or equivalent related field required.
• Three (3+) years previous relevant experience in a Mental or Behavioral Health environment required.
• Ability to use business acumen to exercise good judgment and decision-making.
• Ability to deal with sensitive issues with diplomacy and discretion.
• Must be able to multi task with strong organizational skills.
• Excellent interpersonal skills and able to communicate both verbally and in written form.

Responsibilities

• Housing Liaison: acts as liaison between participants, superintendents and building managers to ensure repairs are completed.
• Case Management and Record Keeping: provides case management and on-site crisis intervention, completes service plans, maintains documentation and progress notes.
• Housing Reporting Requirements: completes annual tenant rent calculation forms and apartment inspections, updates program database and leases.
