Job summary

Responsibilities

• Gather consents, complete screenings and assessments per policy.
• Engage in care coordination and ensure access to comprehensive services.
• Document and update plan of care and notes.
• Engage with housing and social services to support clients.
• Meet with supervisor regularly and participate in team meetings and case conferences.
