Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients at home according to an established care plan. Responsibilities include bathing, feeding, grooming, vital signs, safety, and documentation, with emphasis on clear communication with families and nurses.

Part-time position with flexible hours and competitive salary. Qualified applicants will meet training requirements, hold CPR certification, complete Maxim screenings, and participate in Maxim’s competency

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• CPR certification must be current and maintained.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with daily living and per care plan.
• Baths, oral hygiene, dressing, toileting, feeding, etc.
• Take and record vital signs when ordered (temp, pulse, respiration, BP).
• Safe transfer and ambulation; assist with mobility.
• Exercise according to the plan of care.
• Range of motion and simple ext therapies as ordered.
• Assist with patient laundry and daily activities.
• Maintain safety and proper use of equipment.
• Observe and document patient condition and report changes.
• Communicate clearly with client, family and office.
• Follow plan of care and complete all assigned tasks.
• Observe home safety practices and emergency procedures.
