Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes per a detailed care plan. You will support daily living activities and personal care, while observing safety and reporting changes to the nurse supervisor.

Responsibilities include bathing, hygiene, transfers, vital signs when ordered, and documenting care. The role emphasizes teamwork, adherence to plan, and flexible scheduling within a part-time framework.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assists patients with bathing, oral hygiene, dressing, toileting, feeding as per care plan.
• Take and record temperatures, pulse, respiration and blood pressure per orders.
• Safe transfer and ambulation as instructed.
• Assist with prescribed exercises according to the plan of care.
• Observe and document patient condition and report changes to the assigned nurse supervisor.
• Follow care plan and complete all assigned tasks.
• Ensure home safety and use equipment safely (foot stools, side rails), and observe safety practices.
