Job summary

People Care is hiring three Bilingual Home Care Coordinators to support our home care programs, including DFTA / NYC Aging and NHTD-related coordination. This is an execution-focused, 100% on-site role at our Brooklyn office, requiring bilingual English and Spanish communication and strong organizational skills.

You will own end-to-end case lists, maintain staffing continuity, respond to coverage changes, and document visits and authorizations in our scheduling system.

Qualifications

• Bilingual English and Spanish required.
• 1+ years of home care coordination, scheduling, or related home care operations experience.
• Experience managing multiple client cases, schedules, or service needs.
• Experience responding to call-outs, coverage gaps, scheduling changes, or other time-sensitive service issues.
• Experience with HHAeXchange or a comparable home care scheduling / EVV system.
• Ability to maintain accurate case notes, schedules, authorizations, and operational documentation.
• Clear professional communication with clients, families, caregivers, and internal teams.
• Ability to work 100% on-site at our Brooklyn office.

Responsibilities

• Your assigned case list, end to end.
• Keep authorized clients appropriately staffed, maintain coverage, follow open issues through resolution, and ensure important details do not fall through.
