Job summary

Ultimate Care in Brooklyn, NY is seeking a Home Care Coordinator to schedule and coordinate services for clients, assign caregivers, and manage communications. The role is onsite, Monday–Friday, with overtime possible to support staffing needs.

Ideal candidates are bilingual (Russian/Spanish/Creole and English), with experience in home care coordination and proficiency with scheduling platforms such as HHA eXchange. Competitive pay and benefits provided.

Qualifications

• Prior home care coordination experience (staffing cases).
• Bilingual in Russian/Spanish/Creole and English.

Responsibilities

• Coordinate and schedule home care services based on clients' needs and caregiver availability.
• Assign caregivers based on skills and client compatibility.
• Manage a high volume of incoming calls and resolve inquiries promptly.
• Adapt to scheduling changes due to callouts or emergencies.
• Maintain scheduling records using HHA eXchange.
• Track daily attendance and EVV submissions or caregiver timesheets.
• Call existing patients monthly and document communications using templates.
• Mediate between caregivers, clients, and healthcare professionals for clear communication.
