Neighborhood Health Plan of Rhode Island is seeking a Community Health Coordinator to support the home-based primary care team. You will coordinate patient registrations, update EMR, and assist with outreach while liaising with care teams to ensure access to medical, behavioral, and social services.

Responsibilities include answering calls, scheduling appointments, documenting encounters, and maintaining HIPAA compliance in a patient-centered, team-driven environment.

Competenze

• High school diploma or equivalent.
• 2 years' experience in health care setting.
• Bilingual in Spanish (read, write, speak).
• Strong communication and customer service skills.
• Proficient in EMR documentation and data entry.
