Neighborhood Health Plan of Rhode Island is seeking a Community Care Coordinator to manage and coordinate care for members with social needs. You will work with the health care team, members, and external partners to ensure patient‑centered, cost‑effective care.

Responsibilities include outreach, care coordination under Medical Case Manager, assisting with referrals, documenting activities, and supporting development of community resources.

Qualifications

• Associates degree in human services or equivalent years of background/experience.
• 3–5 years in medical or social services; or at least 6 months in Neighborhood Health Plan of Rhode Island.
• Strong written and verbal communication skills; customer‑service orientation.

Responsabilités

• Outreach to members and conduct initial health/social needs screening.
• Perform HRA and initial health screening.
• Provide telephonic screening of non‑medical needs and coordinate referrals.
