Job summary

Neighborhood Health Plan of Rhode Island is seeking a Community Care Coordinator to manage and coordinate care for members with social needs. You will work with medical teams, members, families, and social service agencies to ensure quality and cost-effective patient-centered care.

The role involves outreach, initial screenings, referrals, and documentation, with potential off-site visits and travel as needed.

Qualifications

• Associates Degree in human service related field preferred or equivalent years of background and experience to equal degree.
• Three to five years experience in medical or social services setting or minimum of six months experience at Neighborhood Health Plan of Rhode Island.
• Experience with behavioral health and/or long-term care needs is preferred.

Responsibilities

• Under the direction of a Medical Case Manager, performs ongoing care coordination activities including outreach, initial health screening, and referrals.
• Provide information to members/families regarding social and community support services.
• Assist with calls to social service agencies and coordinate services.
• Conduct off-site visits to members at home, hospital, or community health centers as needed.
• Maintain comprehensive documentation of activities in CMAQE or approved systems.
• Collaborate with Medical and Integrated Care Managers to coordinate community resources.
