Job summary

Qualifications

• Bachelor's Degree in Arts or Science required; Master's preferred.
• Minimum of 1 year relevant experience in community health, mental health or hospital setting.
• Excellent knowledge of public health paradigms and evidence-based interventions.
• Spanish/English bilingual skills required.

Responsibilities

• Lead PCC program objectives under supervision of the Director.
• Coordinate with Marketing to support program promotions.
• Collect data and prepare reports for monitoring and evaluation.
• Ensure grant compliance and expenditure tracking with Grants Dept.
• Provide direct patient care and referrals to PrEP and behavioral health services.
• Engage with high-risk populations and coordinate cross-agency services.
• Maintain program documentation and protect patient confidentiality.
