Job summary

NeighborHealth Corporation in Boston’s East Boston area seeks a Community Based Prevention Health Coordinator to advance HIV/STD prevention, PrEP linkage, and behavioral health connections. The role blends in-person and virtual outreach, risk assessment, and education within a Latinx LGBTQ-focused context. Bilingual English/Spanish required.

Bachelor’s degree with a Master’s preferred. You’ll coordinate with local agencies, support program reporting, and help ensure compliance across the CBP

Qualifications

• Bachelor’s degree required; Master’s preferred.
• Bilingual (English/Spanish) required.

Responsibilities

• Lead PCC program and achieve program objectives.
• Coordinate with Marketing and Grants departments for outreach and reporting.
• Collect data, prepare reports, monitor and evaluate the program.
• Provide direct patient-facing risk assessment and referrals to PrEP and behavioral health services.
• Maintain confidentiality of patient records and partner with local agencies.
