Job summary

NeighborHealth in Boston’s East Boston neighborhood seeks a Community Based Prevention Health Coordinator to lead its PCC program, conduct risk assessments, and provide PrEP and behavioral health referrals. Candidates should have a Bachelor's degree in a related field, at least 1 year in community health, and bilingual English/Spanish.

Qualifications

• Bachelor’s degree required; Master’s preferred.
• Experience in community health, mental health or hospital setting preferred.
• Strong written and verbal communication skills; ability to assess risk and refer to services.

Responsibilities

• Lead the PCC program and ensure objectives are met.
• Coordinate outreach and marketing with NeighborHealth teams.
• Collect data, prepare reports, and ensure program compliance.
• Facilitate PrEP nights/socials and connect clients to behavioral health services.
• Maintain confidential patient records and build community partnerships.
