Job summary

Matthew 25 AIDS Services in Kentucky seeks a compassionate case manager to provide medical case management under KHCCP, connecting HIV+ individuals with essential services.

You will assess client needs, advocate and educate, develop individualized care plans, promote health literacy, support language access, and help clients access medications and insurance while upholding the program's commitment to equity and community.

Qualifications

• A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred; two years case management experience may qualify.
• Experience working in a nonprofit organization or strong service-driven motivation.
• 100% commitment to Matthew 25's mission, vision and values.
• Flexibility to travel across Kentucky.
• Valid Driver’s License with at least three years driving experience and less than two moving violations in the past three years.
• Must pass a drug screen and background check.

Responsibilities

• Assess and identify the individual needs of clients, including those who speak English as a second language, providing tailored interventions and fostering patient-centered care.
• Advocate, support, and educate clients, focusing on wellness and informed health decisions with culturally sensitive approaches for language barriers.
• Develop, implement, and manage an inclusive, individualized care plan considering language needs and referrals to community resources.
• Promote health literacy by helping clients make informed health choices and addressing language or cultural barriers.
• Strengthen professional practice through ongoing learning of best practices for working with multilingual populations.
• Ensure clients receive certification/recertification for the Kentucky Ryan White Program and facilitate access to medications, treatment adherence counseling, and medical insurance with language support.
