Job summary

Chicanos Por La Causa is hiring a bilingual Medical Case Manager in Phoenix, AZ to help lead the fight against HIV through education and care. You will provide direct services to clients living with HIV, coordinate with community supports, manage appointments, and act as a clinical liaison to medical services.

You will conduct needs assessments, implement treatment plans, facilitate referrals, and communicate effectively in both Spanish and English to help clients navigate barriers to care.

We are hiring a bilingual Medical Case Managerto help lead the fight against HIV through education and care!

CPLC LUCES (Latinos Unidos Contra el Sida/ Latinos United Against AIDS) provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.

LOCATION: 6850 W. Indian School Rd, Phoenix

PAY: $26.92- $29.16 hourly, non-exempt DOE

HOW WILL YOU MAKE AN IMPACT IN THIS ROLE?

• You willprovidedirect covered services to clients living with an HIV diagnosis, coordinatewith community support and stakeholders.
• You will be responsible for managing appointment schedules, assigned caseload as a clinical liaison to medical services, conducting needs assessments, implementing treatment plans, facilitating referrals, and participating in ongoing learning experiences.
• You will provide services in both Spanish and English, helping clients navigate systems and barriers to access.

MINIMUM QUALIFICATIONS AND COMPETENCIES

• Bachelor’s degree from an accredited college in alicensed*field related to case management such as social work, nursing, public health or other human services related fieldpreferred.
• Or high school diploma and comparable professional knowledge, skills and abilities that documents 4 years of experience specific to medical case management (conducting needs assessments, implementing treatment plans, facilitating referrals) may be substituted for thedegree.
• Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and otherSTD’s.
• Bilingual: English/Spanish skills is a requirement. Must be able to communicate to Spanish speaking clients; considering the importance of knowing & communicating medical terminology in bothlanguages.
• Able to work a flexible schedule as needed per programrequirements.
• Must successfully complete the agency’s orientation and demonstrate competency, knowledge, andskills.
• Have good organization and time management skills.

This position requires the ability to work a flexible schedule as needed per programrequirements, including driving out into the community to meet clients where they are.
