Legacy Community Health in Houston is seeking a Medical Case Manager for the Ryan White program to deliver psychosocial support and advocate for HIV+ patients. You will coordinate access to medical and social services and help patients navigate complex care networks.

Qualifications

• Master's degree in social work is required.
• Texas LMSW is required.
• Experience with HIV social services or related populations is preferred.
• Excellent communication and customer service skills.
• Proficient with electronic health record systems and case documentation.

Responsibilities

• Identify and screen HIV+ individuals to address social service needs.
• Provide information and referrals for medical and psychosocial linkages.
• Advocate to reduce service gaps and promote adherence to care.
• Assist patients in reconnecting with essential medical services when lost to care.
• Maintain documentation to meet program standards and timelines.
• Travel between locations to ensure access to services.
• Participate in community meetings and ongoing training.
