Job summary

CAPS & Maple City is a Goshen, IN-based organization delivering health services to children and families. The Health Care Navigator helps patients access Medicaid, the Health Insurance Marketplace, and affordability resources, supporting care access and continuity within the CAPS & Maple City team.

The role requires Indiana Navigator certification, an associate degree in social work or related field, and CAC training. Bilingual English/Spanish and strong interpersonal skills are essential.

Qualifications

• Experience working with diverse groups of people.
• Ability to translate real-life situations into Medicaid framework.
• Fluently bilingual in English and Spanish; oral and written.

Responsibilities

• Assists patients in accessing insurance information and coverage through Medicaid and the Health Insurance Marketplace.
• Maintains certification as an Indiana Navigator.
• Assist patients in accessing in-house affordability resources.
• Assist patients access health care financial assistance programs through Goshen Health.
• Coordinates and follows up with care team to support patient access and continuity of care.
• Documents patient contact and interactions in the electronic medical record (EMR).
• Utilizes EMR and telephone encounters to accurately capture patient contacts and services.
