Job summary

Kinetic Personnel Group is recruiting a Social Worker for a PACE program in Garden Grove, Orange County. This onsite role requires a Master’s in Social Work, bilingual Spanish/English, and 1 year working with gerontology populations.

The position involves IDT participation, care planning, and occasional home visits; travel within Orange County is expected. Pay ranges from $32 to $52 per hour, DOE, with a six-month temporary period before conversion to a permanent role.

Qualifications

• Master’s degree in social work from an accredited program.
• 2 years of experience in a health-related field; elderly population preferred.
• Bilingual in Spanish and English; CPR/First Aid certification within 6 months.

Responsibilities

• Participates in IDT assessments and care planning for PACE participants.
• Provides care coordination, discharge planning, and home visits as needed.
• Advocates for participants and connects with community resources.
