Job summary

Suvida Healthcare is seeking a bilingual (English/Spanish) Physical Therapist to join our integrated team serving Medicare-eligible Hispanic seniors. You’ll provide patient-centered PT services, focusing on movement optimization, pain management, and functional outcomes within our primary care program.

The role requires a DPT or equivalent, Arizona licensure, and 3+ years of PT experience, preferably with geriatric patients.

Qualifications

• DPT degree or equivalent in Physical Therapy.
• Active license by the Arizona Board of Physical Therapy Examiners, or the ability to obtain one.
• Current BLS Certification

Responsibilities

• Conduct thorough patient evaluations using clinical observation and standardized testing.
• Develop individualized, evidence-based care plans to improve function.
• Provide patient-centered education to improve movement and activity.
• Supervise PTAs and PT interns, and collaborate with care teams.
• Lead fall prevention classes and complete fall risk screenings.
• Integrate Suvida clinical programs into practice workflows to optimize patient experience.
