Job summary

Suvida Healthcare LLC in Phoenix, Arizona seeks a bilingual Physical Therapist to join our team providing patient-centered PT services for Medicare-eligible seniors. You will lead evaluations, craft evidence-based care plans, educate patients and caregivers, and collaborate with interdisciplinary teams to optimize mobility and quality of life.

Expect a manageable caseload with 1-on-1 patient care, built-in documentation time, and a culture that values equity and compassionate service.

Qualifications

• 3+ years of physical therapist experience.
• Geriatric experience preferred.
• Bilingual English and Spanish preferred/required.
• Advanced Microsoft Office tools (Word, Excel, PowerPoint).

Responsibilities

• Conduct thorough patient evaluations using observation and testing to identify PT needs.
• Develop individualized, evidence-based care plans to improve function.
• Provide patient-centered education to improve movement and activity.
• Utilize technology to support patients and caregivers.
• Ensure proper use of equipment and safe handling guidance.
• Supervise PTAs and PT interns.
• Collaborate with care teams to set patient-specific goals.
• Deliver high-quality, empathetic patient experiences.
