Job summary

Suvida Healthcare is seeking a licensed Physical Therapist to join our integrated care team serving Medicare‑eligible Hispanic seniors. You will deliver patient-centered physical therapy services and movement programs to improve function, manage pain, and enhance quality of care.

With 3+ years of experience, bilingual English/Spanish preferred, you will supervise PTAs and collaborate with interdisciplinary teams to tailor care plans and promote outcomes.

Qualifications

• DPT degree from an accredited program.
• Active license by the Arizona Board of Physical Therapy Examiners, or ability to obtain one.
• Current BLS Certification.

Responsibilities

• Conduct thorough evaluations of patients using clinical observation and testing to identify needs.
• Develop individualized, evidence-based care plans to improve function and participation.
• Provide patient-centered education to improve movement and activity.
• Supervise and direct PTAs and PT interns.
• Collaborate with interdisciplinary teams to set patient-specific goals.
• Lead fall prevention classes and complete risk screenings.
• Contribute to quality improvement and ensure a positive patient experience.
