Job summary

Responsibilities include evaluations, evidence-based care plans, patient education, supervision of PTAs, collaboration with interdisciplinary teams, and leading fall-prevention programs, with a manageable caseload.

Qualifications

• 3+ years of physical therapy experience.
• Bilingual English/Spanish required.
• Geriatric experience preferred.
• Active Texas license or ability to obtain one.

Responsibilities

• Conduct thorough evaluations to identify PT needs.
• Develop individualized, evidence-based care plans.
• Provide patient-centered education to improve movement.
• Supervise PTAs and PT interns as needed.
• Collaborate with interdisciplinary teams to set goals.
• Lead fall-prevention classes and screenings.
• Integrate Suvida clinical programs into practice workflows.
