Job summary

Wheeler Clinic in Waterbury, CT seeks a Bilingual Family Support Specialist for the Foster Care program. You will manage a caseload, coordinate care with DCF, schools and other supports, and facilitate groups as needed.

The role requires a BA/BS in a relevant field, strong language skills in English/Spanish, and experience with mental health or co-occurring substance use disorders. Full-time, on-site with comprehensive benefits.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required.
• Experience working with clients with mental health and/or co-occurring substance use disorders.
• Valid driver’s license and ability to operate a vehicle in the community.

Responsibilities

• Develops case management plans or plans of care and provides direct case management services and/or coordination of care for each client/family.
• Provides front line crisis support to individuals and/or families by phone, in-home and/or in the community as needed.
• Provides group, individual and/or family counseling with supervision and coordinates treatment phases with providers and family supports.
• Maintains records of counseling, coordination of care and case management activities for reporting and compliance with Medicaid/licensing requirements.
