Job summary

United States Digital Space LLC is expanding a field-based care management role in Alameda County. You will deliver hands-on support in patients’ homes and communities, onboard new patients, and help overcome barriers to access.

Applicants must be bilingual (English/Spanish) and able to work in the field five days a week. The role emphasizes strong organization, independent work habits, and collaboration with nurses, behavioral health, and social support teams.

Qualifications

• Bilingual English/Spanish required for community engagement.
• 1+ years of field-based case management or care coordination.
• Must have reliable transportation and driver’s license with insurance.
• Strong organization and ability to manage a busy field schedule.
• Comfort with Google Suite, Slack, CRM, and basic MS apps.

Responsibilities

• Manage a high-volume field caseload and ~30 in-person visits weekly.
• Travel throughout Alameda County meeting patients where they are.
• Support community events and coordinate care for complex needs.
• Serve as primary contact for members with medical, behavioral, social needs.
• Meet patients at homes, clinics, or partner sites for onboarding and care packages.
• Document interactions and coordinate referrals across care teams.
