Pair Team in Oakland, CA is seeking a Field-Based Care Coordinator to bring care directly to patients in their homes and communities. You will manage a high-volume caseload, perform in-person visits, and coordinate with nurses and behavioral health staff to ensure timely access to services.

This role involves daily travel within Alameda County, scheduling flexibility, and hands-on support to overcome barriers to care.

Qualifications

• 1+ years of field-based (in-person) case management or care coordination.
• Live near Alameda County and familiar with local resources.
• Reliable transportation and a valid driver’s license.
• Strong cultural fluency and ability to build trust with diverse populations.
• Empathy, patience, and ability to work in multidisciplinary teams.
• Excellent organizational skills and attention to detail.
• Proficient with Google Suite, Slack, CRM, and basic Excel/Word.

Responsibilities

• Manage a high-volume field-based caseload with ~30 in-person visits weekly.
• Travel across Alameda County to meet patients where they are.
• Support community events and care coordination for complex needs.
• Serve as primary contact for members with complex medical, behavioral, and social needs.
• Meet patients at homes or community sites to onboard and deliver care.
• Document interactions and maintain care plans.
• Coordinate services across primary care, behavioral health, and social supports.
• Collaborate with nurses and care managers to ensure continuity.
• Identify barriers to care and address transportation or access issues.
• Advocate for member needs within healthcare and social systems.
• Participate in care team meetings to review panels and plans.
