Job summary

Pair Team is expanding its field-based care management program in Alameda County. The Field Care Coordinator will provide hands-on, in-home and community care coordination, managing a busy schedule and addressing barriers to access.

You’ll support onboarding, coordinate with nurses and care managers, and help patients navigate resources, while traveling across the county five days a week.

Qualifications

• 1+ years field-based in-person case management or care coordination.
• Reliable transportation for regular field visits.
• Valid driver’s license and auto liability insurance.
• Strong cultural fluency and empathy for diverse populations.

Responsibilities

• Manage high-volume field-based caseload with ~30 visits/week.
• Meet patients in homes or community sites across Alameda County.
• Coordinate services across primary care, behavioral health, and social resources.
• Document interactions and care coordination activities accurately.
• Advocate for member needs within healthcare and social support systems.
