Job summary

Pair Team is expanding its field-based care management efforts in Alameda County. The Field Care Coordinator will bring hands-on support directly to patients in homes and communities, managing a high-volume caseload and completing approximately 30 in-person visits per week.

You’ll work across community settings to deliver care packages, onboard new patients, and coordinate with care teams. The role requires 1+ years of field-based experience, reliable transportation, a valid driver's license,

Qualifications

• 1+ years of field-based in-person case management or care coordination.
• Physical location in or near Alameda County with local familiarity.
• Reliable transportation for regular field visits (homes, clinics, partner sites).
• Valid driver’s license and auto liability insurance.
• Strong cultural fluency and ability to build trust with diverse populations.
• High empathy and ability to stay patient under adversity.
• Ability to work collaboratively in a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Familiarity with Google Suite, Slack, CRM databases, and basic Excel/Word/email/video tools.

Responsibilities

• Manage a high-volume field-based caseload with ~30 in-person visits per week (up to 120 monthly).
• Travel throughout Alameda County to meet patients where they are.
• Support community events and care coordination for complex needs.
• Serve as primary contact for members with complex medical, behavioral, and social needs.
• Meet members at homes, communities, or partner sites for onboarding and care delivery.
• Conduct in-person visits to assess needs, support adherence to care plans, and document activities.
• Coordinate services and referrals across primary care, behavioral health, and social resources.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure continuity of care.
• Identify barriers to care (transportation, medications, health literacy) and address them.
• Develop individualized care plans and ensure timely follow-up on interventions.
• Maintain accurate documentation of member interactions and care coordination.
• Advocate for member needs within healthcare and social support systems.
• Participate in care team meetings to review panels and align on plans.
