Job summary

Brooklyn Community Services in Brooklyn, NY seeks a Case Manager (Family Development Coach) who is bilingual in Spanish to provide direct case management for fathers enrolled in the Fatherhood Initiative.

You will conduct intake, assess needs, develop Individual Service Plans, meet monthly to support goals, and coordinate referrals to community resources while maintaining confidentiality and building trusting relationships.

Qualifications

• Bachelor's in social work or related field with City systems experience; or associate degree with Family Development Training.
• Experience with City systems (public assistance, child support, welfare, housing/education) or completion of Family Development Training within one year.
• 3+ years case management/counseling experience with City systems (public assistance, etc.).
• Ability to build and maintain trusting relationships.
• Ability to maintain confidentiality.
• Sensitivity to the needs of program participants and graduates.
• Experience recruiting fathers and sustaining engagement.
• Experience in Maternal and Child Health is a plus.
• Experience with diverse populations and organizations.
• Strong interpersonal and organizational skills.
• Strong written and verbal communication skills.

Responsibilities

• Conduct participant intake, assessment, and enrollment activities.
• Develop and monitor Individual Service Plans for each participant.
• Meet with participants individually on a monthly basis to support goal attainment.
• Provide referrals and coordinate service linkages to community resources.
• Support participants during program participation and for a minimum of three months post-exit.
• Track attendance, participation, and progress in required data systems.
• Utilize approved case management tools and frameworks.
• Collaborate with Training Specialists to align case management with group programming.
• Maintain accurate and timely documentation of all participant interactions and services.
• Other duties as assigned.
