Job summary

Responsibilities

• Conduct outreach and screening to identify families eligible for home visiting services.
• Initiate and maintain regular contact with families, primarily via visiting them in their homes.
• Document initial family assessments and use assessment tools as indicated.
• Interview parents prenatally and/or shortly after birth.
• Review hospital or clinic records and maintain timely documentation in the electronic database.
• Assist in making and attending health and human service appointments.
• Support parents to improve skills for a safe, nurturing home environment.
