Qualifikationen

• Spanish language proficiency required for serving Spanish-speaking clients.
• Minimum 2 years of case management experience with at-risk children and families.
• Valid driver's license and ability to drive with Buckner’s insurance coverage.
• Ability to lift up to 50 pounds occasionally and up to 20 pounds frequently.
• Must be 21 years or older to drive on Buckner’s behalf.
• Ability to walk, stand, and sit for prolonged periods.

Aufgaben

• Create and implement Family Hope Center programs aligned with core programs and the Family Strengthening Framework.
• Coordinate supportive services for families and design services to meet goals.
• Assist with out-of-home placement or respite care arrangements as needed.
• Compile intake and referral information and complete enrollment paperwork.
• Provide case management including updating service plans and referrals.
• Coordinate therapy services and offer crisis intervention counseling as appropriate.
• Help participants access community resources and enroll in services.
• Develop relationships with community institutions to coordinate services.
• Analyze program needs and implement changes to meet client needs.
• Create and manage event schedules and conduct trainings for clients and groups.
